6.3 Family Functioning Assessment/Reassessment (CD-14A)
If the case should remain open, a CD-14A and a new Written Service Agreement (CD-14 B) is due within 30 days of the expiration of the previous treatment period.
At reassessment, the Children’s Service Worker will complete the Family Functioning Assessment/Reassessment (CD-14A) and the Risk Reassessment (CS-16E). The worker will be able to note progress or new concerns by comparing the new CD-14A to the previous one. The reassessment progress should focus on:
- Enhancing caregiver protective capacity and thereby reducing the probability for future child maltreatment;
- Identifying service needs that were not addressed in the previous assessment or met in the previous treatment plan; and
- Identifying other service needs that have arisen since the previous assessment which may contribute to risk.
