National Medical Support Notice
Parts of Notice

The National Medical Support Notice is divided into two parts, with response sections and instructions for each part:

1. Part A - Notice to Withhold for Health Care Coverage -Child Support Enforcement (CSE) or another state's child support enforcement agency completes this document and sends it and Part B to the employer.
   
   Employer Response - This section gives the employer the opportunity to directly respond to Part A under certain conditions.
   
   
2. Part B - Medical Support Notice to Plan Administrator - The employer forwards this part to the employee's health care plan administrator.
   
   Plan Administrator Response - The plan administrator completes this section and returns it to CSE or other state's child support agency.
