Chronic Care Improvement Program

APS Healthcare is working with the MO HealthNet Division to support physicians and assist chronically ill MO HealthNet participants enrolled in Missouri's Chronic Care Improvement Program (CCIP). The focus of the program is to improve the quality of care for participants with chronic illness, decrease complications associated with chronic illness and reduce costs associated with delivering care to this population.

APS provides education, health management support services and improved coordination of healthcare services for the MO HealthNet Division — MO HealthNet participants who suffer from long-term chronic diseases, including diabetes, asthma, chronic obstructive pulmonary disease (COPD), gastroesophageal reflux disease (GERD), cardiovascular disease and sickle cell disease.

As part of the CCIP program, APS deployed the first state Internet-based plan of care, APS CareConnection® for health and disease management. The Internet-based plan of care enables the participants, providers and APS health coaches to collaborate more effectively using electronic medical records.
