All EUs must have a mid-certification review (MCR).

The FA546 Food Stamp Mid-Certification Review Report form asks the EU to report the following:

Any changes reported on the FA546, are acted upon when completing the MCR.

An FA546 is generated by FAMIS on the 5th day of the month prior to the MCR month. The FA546 is due from the EU the last day of the month prior to the MCR month. If the EU has not returned the MCR by the last day of the month prior to the MCR month, an Informational Notice (FA-601) will be sent to the EU on the first day of the MCR month.

The reminder notice will inform the EU to return the form within 10 days from the date the notice is mailed. If the form is still not received from the EU and an Audit Date is not entered in the FAMIS system at the close of business on the 10th day, an Adverse Action (FA-150) notice will be sent to the EU for closure of the case.

EXAMPLE: Ms. Jones' Food Stamp certification is July 2017 – June 2018. The MCR month is December 2017. FAMIS sends the EU a FA546 on November 5, 2017. The FA546 is due back from the EU before November 30, 2017.

If Ms. Jones MCR has not been received and an audit date entered in FAMIS on or before November 30, 2017, an Informational Notice (FA 601) will be mailed to the EU on December 1, 2017 reminding Ms. Jones that the MCR is due back on or before December 10, 2017. If Ms. Jones still doesn’t return the MCR form by December 10, 2017 an Adverse Action notice will be generated to close the case on December 21, 2017.

The alert "Mid-Certification Review Required." is generated at the time the FA546 is mailed to the EU. The alert continues to display until the review action is taken in FAMIS. The alert elevates to the supervisor on the first day of the MCR Month.

Mid-Certification Reviews may be initiated by the eligibility specialist when FAMIS cannot generate the FA546 because of timing. Cases that are in closed status on the 5th of the month, and are cancel closed in the month prior to the MCR month require the eligibility specialist to initiate the MCR process. The same mid-certification review procedures apply.

EXAMPLE: Ms. Jones' Food Stamp certification is June 2012 – May 2013. The MCR month is November 2012. The EU is closed on October 2, but is cancel closed on October 7. Because the case was in closed status, the FA546 MCR Report is not sent on the 5th of October. The ES must generate the FA546 MCR Report when the case is cancel closed. The FA546 MCR Report is due back from the EU before November 31, 2012.

To create a review, go to the TA/MA/FS Review History (FM57) screen in FAMIS, prompt for Food Stamp program and press F14=ADDREIN. The message, "REIN Created, Notice Scheduled for Printing" displays at the bottom of the screen.

NOTE: Reviews created by the eligibility specialist on-line can be deleted on the same day the review is created as long as no factors have been reviewed. Enter "D" on the command line and press enter to delete. After the date of creation has passed the review must be completed and cannot be deleted.

The FA546 is not to be used in place of the FA331 Appointment Letter/Request for Contact. When an EU's circumstances are in question or information is received that requires additional clarification, send an FA331 to request contact from the EU.
The FA546 is considered returned even if the information provided is incomplete or unclear. Enter the date the FA546 is received on the Review Audit Detail (FM55) screen.

Complete all MCR review factors in FAMIS when the FA546 is returned. Print and review IMES for any individual age 16 or over. Print and review IIVE for all EU members. Manually access and update the Training/Work Requirement (WORKREQ/FMMR) screen for mid-certification reviews. When all factors have been reviewed and the status is CMP, complete an eligibility determination (EDRES). Authorize the action.

Follow current verification procedures for reported changes. If additional verification is required to determine continued eligibility, leave the verification code blank and complete the REINUPD function. FAMIS creates outstanding verification and generates the FA325 which is mailed to the individual. If additional information and/or verification regarding the FA325 is not provided, FAMIS generates an Adverse Action Notice for failure to provide requested information.

Applications for other Income Maintenance programs in FAMIS may be used to complete a mid-certification review for Food Stamps. This is not processed automatically by FAMIS. The date the other program application is received is entered as the notice received date on the Review Audit Detail (FM55) screen. Complete all MCR review factors in FAMIS.

A Food Stamp add-a-person application completes all of the factors required for an MCR. FAMIS completes the MCR when the action is authorized and assigns the EU a new review date.

EXAMPLE: Ms. Jones' Food Stamp certification is July 2012 – June 2013. The MCR month is December 2012. The FA546 is generated on November 5, 2012. The FA546 is due back from the EU before November 30, 2012. On November 15, 2012, the EU reports a new member and the add-a-person application is authorized on November 16, 2012. This completes the MCR.

A Food Stamp recertification completes all of the factors required for an MCR.

EXAMPLE: Ms. Jones' Food Stamp certification is June 2012 – May 2014. In October 2013 earned income is added to a current member. The certification does not change, but an MCR is due six months later, April 2014, which is just one month before the certification ends. (Changing the MCR is discussed in the next section.) The FA546 MCR Report is generated March 5, 2014. The EU comes to the office to discuss the MCR. Since it is less than 60 days from the end of the certification, the eligibility specialist has the EU complete an application for Food Stamp benefits, completes the interview for the recertification, and authorizes the action. Completing the recertification also completes the MCR.

When the EU is closed for failure to return the FA546, cancel close the EU if the form is received before the end of the MCR month. Benefits are not prorated.

EXAMPLE: EU with earned income certified June 2012 through May 2013. The FA546 MCR Report is mailed October 5, 2012. The FA546 MCR Report is not received by October 31. The FA-601 Information Notice is sent November 1, 2012. The MCR Report is still not received 10 days later. The FA 510 Adverse Action Notice is sent on November 12, 2012 and case is closed November 22. The FA546 MCR Report is received November 25, 2012. Cancel close this EU. FAMIS issues a full month's benefit for November.

If the FA546 MCR Report is not received before the end of the MCR month, the EU must reapply for Food Stamp Benefits.

1140.020.05 Mid-certification Review for FS Only Cases

Enter the date the mid-certification review form is received by the agency on the Review Audit Detail (FM55) screen.  Complete the Food Stamp MCR flow and update eligibility factors with all reported changes.  Generate an FA-325 for missing or incomplete information needed to complete the required review factors.  Provide 10 days to complete and return the information.  If the EU is does not provide the needed information, an adverse action is sent to the EU notifying them that their case will be closed for failure to cooperate.

1140.020.10 Mid-certification Review FS/IM Combination Cases

When an individual has a mid-certification review and a review for another program due at the same time, they will receive both forms in the mail. If the Food Stamp MCR is not returned but the review for the other program form is, enter the date the IM review form is received for the Food Stamp MCR on the Review Audit Detail (FM55) screen. Contact the EU to discuss the required review factors for the Food Stamp MCR. Complete the Food Stamp MCR flow and update information received. Generate an FA-325 for missing or incomplete information needed to complete the required review factors for the Food Stamp MCR.

When the eligibility factors have not changed, record the date the FA546 was received and a statement regarding no changes were reported on the Eligibility Unit Member Role (EUMEMROL/FM3Z) screen.

EXAMPLE: Mid-Cert Review: Received FA546 on February 1st. Mr. X stated household composition is he and Mrs. X. There are no changes in income of $534 SSA and expenses previously budgeted. Resources are checking account $800, and a 1984 Ford Tempo. No further action is needed.

When an eligibility factor changes, record on the Eligibility Unit Member Role (EUMEMROL/FM3Z) screen a summary of the review findings. Record on the appropriate detail screens what information changed and information regarding the verification. When a new eligibility determination is run, record on the Action Authorization (FM3H) screen that the action is due to a mid-certification review.

EXAMPLE: Record on the Eligibility Unit Member role (EUMEMROL/FM3Z) screen: Mrs. M completed a FA546 on August 1st. New resource - received $20,000 insurance settlement July 2nd. No other changes reported.

Record on the Liquid Resource (FMW0) screen: Insurance settlement verified by letter sent to Mrs. M from the insurance company, dated July 2nd.

Record on the Action Authorization (FM3H) screen: Notice of adverse action sent as result of a mid-certification review. Mrs. M received $20,000 insurance settlement.

1140.020.20 Time Frame for Acting on Changes for Mid-Certification Reviews

When a change is discovered or reported at a mid-certification review, within ten days:

If verification is not required, authorize the actions. FAMIS notifies the EU if benefits remain the same or increase, or will inform the EU of an adverse action.
