The DFS GED Billing Form
has been revised as of March 2004 and is now entitled FSD GED BILLING
The revised form is electronic
The administration fee will continue to be direct billed by Department of Elementary and Secondary Education (DESE) to the Family Support Division (FSD). The administration fee will be paid for individuals case managed by FSD who are in an educational component in the JOBS system. The testing center fee is to be paid by the individual taking the GED test directly to the local testing center. A column for the individual’s birth date has been added to the form next to DCN.

The statement “Attach School Permission with Application for Missouri Certificate of High School Equivalence (GED) when applicable” has been added to the informational box. (See School Permission box on application to determine when applicable.)
FSD Central Office staff will review and authorize the payment. To accomplish this, additional fields are added to the bottom of the form.
