Effective immediately, IM-110 Replacement Request forms must be sent to the Food Stamp Program and Policy Unit for the replacement determination to be made. These forms and the IM-113 Replacement Request/Affidavit for Food Stamp Benefits Lost from an EBT Account, along with any accompanying documentation, can be faxed to 573-522-4333 or scanned and sent via email to Rachel.E.Morris@dss.mo.gov. County staff can no longer enter the REP payroll type code on the Payment Maintenance (FM5J) screen.
If a disaster strikes and there is a need to issue replacement benefits for large numbers of EUs, instructions will be sent to county staff.
The Food Stamp Manual sections regarding Replacement Issuances are revised to reflect the new process.
