Subject:  Eligibility Determination

 

Please find attached the document which must be utilized when negotiating an extension for Status "02" cases beyond the normally accepted 60-day period.

 

This document must be sent to the consumer or the consumer’s guardian or representative and signed/dated by the appropriate individual. When completed by the consumer this document must be returned to the appropriate district office. All consumers should be provided a self-addressed, stamped envelope for return mailing purposes.

 

The RSB staff person must have discussed the need for the extension and the consumer must agree with the extension. Copies of the extension letter must be in the case record, signed and dated by the consumer and agency staff person. Please ensure that a copy of this document is sent to Sally Howard in order that we may utilize this information statistically.

If you have any questions regarding this matter let us know as soon as possible.

This letter is to confirm our discussion that there is a need to extend the date Rehabilitation Services for the Blind (RSB) will complete your determination of eligibility.

We agree and this letter confirms that exceptional and unforeseen circumstances beyond the control of the agency exist that prevent RSB from completing the determination of eligibility within the sixty-day period.
