MEMORANDUM FOR DISTRICT SUPERVISORS, VOCATIONAL REHABILITATION COUNSELORS AND BILL-PAYING STAFF

SUBJECT:  Facilities at a Glance Document and Facility Selection Form

Effective June 15, 2006, any eligible individual who has a rehabilitation need for and is considering attendance at a PVA facility should be provided with the current PVA Facilities at a Glance document attached to this memorandum (dated 5-25-06).  They should also review other current information regarding the available facilities, such as the results of any current consumer satisfaction surveys, reasonable and cost-effective telephone or in-person interviews with the facilities, or a review of available consumer handbooks from the facilities prior to making their decision.   

Once the eligible individual has made a selection, and the counselor supports that decision, the eligible individual and the counselor should sign the Facility Selection Form (Page 2 of the attached document) and place it in the case record.  It should be attached to the first authorization for PVA facility services in the bill section.  Copies of the Facility Selection Form are no longer required to be sent to State Office. 

District offices should ensure that they have deleted or destroyed all older versions of the PVA Facilities at a Glance document and Facility Selection Forms.
