(A) Certification of Eligibility. As soon as RSB determines that an applicant is eligible for vocational rehabilitation services, RSB shall complete a certification of eligibility. The certification shall include the statement that the applicant meets the basic conditions of eligibility stated in subsection (3)(B). An appropriate RSB staff member shall sign and date the certification.
(B) Certification for Extended Evaluation. As soon as RSB determines that an applicant meets the requirements stated in subsection (8)(A), an appropriate RSB staff member shall complete, sign and date a certification for extended evaluation.
(C) Certification of Ineligibility. As soon as RSB determines that the applicant or eligible individual is not eligible for vocational rehabilitation services, an appropriate RSB staff member shall complete, sign and date a certification of ineligibility. RSB shall assure that the certification states the reasons for the ineligibility determination. RSB shall carry out all activities stated in subsections (20)(B) and (C).
