Signature Requirements for Division of Developmental Disabilities Person-Centered Service Plans
All Department of Mental Health (DMH), Division of Developmental Disabilities (DDD) Home and Community-Based Services (HCBS) Person-Centered Service Plans (PCSPs) must be approved and signed by the individual and/or their legally responsible person before the PCSP is implemented. This requirement applies to all initial, annual, and amended PCSPs that modify HCBS services, supports, responsible persons, legal status changes, rights, providers, back-up plans, goals, or outcomes.
There are no exceptions to this requirement.
Signature Requirements:
- Signatures may be handwritten or electronic
- Electronic signatures must meet the standards outlined in 13 CSR 65-3.050 and comply with the Missouri Medicaid Audit & Compliance (MMAC) Electronic Signature Requirements
This requirement ensures that individuals receiving HCBS services and/or their legally responsible persons give informed consent of the services, supports, and plans outlined in the PCSP prior to implementation.
