Application for Child Support Services
The Family Support Division’s Child Support (CS) program provides a variety of services, which are described in the brochures “What’s This About Child Support?” and “Does Your Child Have A Legal Father?” Please consider reviewing these brochures to find the CS service right for you.
To apply for CS services, you must print and complete the application forms, then mail the forms to your nearest CS county office.
The CS application forms (PDF, 9 pages, 1.28MB) are:
- Your Rights and Responsibilities as a Recipient of Child Support Services — Informational document to be read and retained for your reference.
- Notice to Non-TANF Applicants: Domestic Violence Services — If you have an issue with domestic violence, CS may have information that will help you.
- Application for Child Support Services — Four-page document allows you to select the specific CS services you need and provide vital information that CS needs to assist you.
- Notice of Privacy Practices Regarding Your Protected Health Information — Informational document to be read and retained for your reference.
When you have completed the CS application forms and mailed them to your nearest CS office — click here to obtain the office address — CS will use the information provided to initiate a case. You will be sent a letter when the case is opened, providing you with contact information and a case number. Please keep this notice available for future reference.
