MHD provides the below documentation templates as a convenience to providers. These are not forms to be filled out, but are examples of acceptable templates for behavioral health assessments, treatment plans, and progress notes.
Effective for dates of service on or after October 1, 2024, doula services are available to all MO HealthNet (Medicaid) enrolled pregnant women as preventive services when recommended by a physician or other licensed practitioner of the healing arts. This includes prenatal, during delivery, and throughout the 12-month postpartum period. Doula services utilize trained community-based doulas with the aim of improving a range of maternal and infant health outcomes by enhancing the participants relevant knowledge and encouraging healthy behaviors.

Not all healthcare providers who are enrolled with MO HealthNet may be taking new patients. You must contact the provider and ask if they are taking MO HealthNet patients.
Medicaid expansion changed the eligibility requirements for Missouri’s Medicaid program (MO HealthNet) to include adults between the ages of 19 and 64 who are eligible for the program. Missouri voters approved Medicaid expansion, the Supreme Court of Missouri held the expansion amendment to be constitutional, and the trial court ordered the state not to prohibit enrollment.
Missouri Medicaid Audit and Compliance (MMAC) is responsible for administering and managing Medicaid (MO HealthNet) audit and compliance initiatives and managing and administering provider enrollment contracts under the MO HealthNet program. MMAC is charged with detecting, investigating and preventing fraud, waste and abuse of MO HealthNet.

In accordance with the Get the Lead Out of School Drinking Water Act (RSMo 160.077), all schools in Missouri are required to test water outlets used for drinking, food preparation, and cooking or cleaning utensils for lead contamination.
The Developmental Disabilities Health Home (DD Health Home) is intended for individuals statewide served through the Division of Developmental Disabilities (DD) who have a qualifying chronic health condition or are at higher risk of health issues or destabilization. The DD Health Home provides population health management and assists individuals with meeting their health and wellness goals while ensuring they get the care they need.
