Targeted case management services are provided to assist individuals in gaining access to needed medical, social, educational, and other services. The individual must reside in a community setting or be in the process of transitioning to a community setting.
The MO HealthNet Division may reimburse school districts for providing transportation services to a child that is receiving an Individualized Education Plan (IEP) direct service and who has a need for specialized transportation.
The MO HealthNet Division may reimburse school districts for providing services that are provided to a MO HealthNet eligible child that are medically necessary and are included in the child's Individualized Education Plan (IEP). Direct services include audiology, occupational therapy, speech therapy, physical therapy, private duty nursing, personal care and behavioral health services.
The MO HealthNet Division covers the cost of visits between Rural Health Clinic (RHC) patients and RHC core practitioners including physician, nurse practitioner, nurse midwife, physician assistant, licensed clinical social worker, licensed master social worker, clinical psychologist or provisionally licensed psychologist.
The RBM program encourages the use of evidence-based, quality imaging practices and will continue to precertify high-tech and cardiac imaging procedure codes.
The MO HealthNet Rehabilitation Center Program pays for the adaptive training of MO HealthNet participants who receive a prosthetic/orthotic device. In addition, rehabilitation centers may provide physical, occupational and speech therapy to children under the age of 21 when medically necessary.
Private duty nursing is the provision of individual and continuous care provided under the direction of the participant’s physician, by a licensed nurse acting within the scope of the Missouri Nurse Practice Act according to an individualized Plan of Care signed by the ordering practitioner. Services within the MO HealthNet Private Duty Nursing Program include shift care by a registered nurse or licensed practical nurse.
MO HealthNet's Primary Care Health Home (PCHH) initiative strives to provide intensive care coordination and care management as well as address social determinants of health for a medically complex population.
The MO HealthNet Division covers the cost of visits with physicians, physician assistants, assistant physicians, chiropractors, and nurse practitioners for eligible participants.
The Missouri Title XIX (Medicaid) Personal Care Program offers medically related services designed to meet the maintenance needs of participants with a chronic, stable condition. Available services include basic and advanced personal care, personal care assistance consumer-directed services, and authorized nurse visits.
