There are several ways you can work with the Rural Health Transformation Program (RHTP).
- Understand how RHTP and ToRCH Care are intended to strengthen rural healthcare
access, sustainability, and outcomes - Use Hub-connected services as they become available, including care coordination,
referrals, transportation supports, behavioral health access, community-based programs - Share feedback with local Hubs, providers, or community partners about barriers to care
- Stay informed on new services, local programs, and community health resources
