What ToRCH Care Means for You
Providers will leverage the hub to make sure the people and organizations in your community work together to support your health and well-being. It is designed so that wherever you enter the system, you can be connected to the help you need.
You might start at a clinic, pharmacy, school, emergency visit, or community organization
If you need help beyond that setting, medical, behavioral, or social, your point of contact will leverage the Hub to connect services together
If chronic conditions exist, new methods for care management may be deployed, such as remote patient monitoring or telehealth
Community health workers (CHWs) or other partners may follow up to make sure you received support
Information is shared (safely and appropriately) so you don’t have to repeat your story at every stop

There are several ways you can work with the Rural Health Transformation Program (RHTP).
- Understand how RHTP and ToRCH Care are intended to strengthen rural healthcare
access, sustainability, and outcomes - Use Hub-connected services as they become available, including care coordination,
referrals, transportation supports, behavioral health access, community-based programs - Share feedback with local Hubs, providers, or community partners about barriers to care
- Stay informed on new services, local programs, and community health resources
