How Can I Get Involved?
- Facilitate selection of local programs that address community needs and share data and
workflow feedback to track program progress and support continuous improvement - Collaborate toward Hub goals, including closed-loop referrals, APM participation, and
selection of local initiatives - Connect to the community information exchange (CIE), participate in APMs, and complete
required workflow training and onboarding - Communicate key directives, strategies, successes, and challenges from the RCN or
RHTO to Hub partners to support effective community-driven healthcare transformation.
