What ToRCH Care Means for You
The Hub is the organizing structure that brings together public health agencies, EMS1, pharmacies, non-clinical providers, all types of healthcare providers, and community organizations across multiple counties to work as one coordinated system.
Hub Anchors:
As the Hub Anchor, the organization serves as the neutral coordinator of this system.
The Hub Anchor is responsible for convening partners across its region and leading its community in creating change in the local healthcare system.
The Hub Anchor hosts infrastructure, staff, governance, and coordination, and ensures partners can work together effectively.
This includes convening a local Leadership Board, supporting shared decision-making, and managing the day-to-day operations that keep coordination moving.
Other Providers:
Providers who are not Anchors remain core contributors in setting priorities, shaping workflows, and delivering care.
Non-Anchor providers help progress decision making, support their community, and drive progress through the Leadership Board.
Provider engagement within the Hub helps reduce fragmentation experienced by all providers (clinical and non-clinical) by coordinating patient referrals, ensuring follow-up, and making communication clearer and more reliable.
How You Can Get Involved
- Facilitate selection of local programs that address community needs and share data and
workflow feedback to track program progress and support continuous improvement - Collaborate toward Hub goals, including closed-loop referrals, APM participation, and
selection of local initiatives - Connect to the community information exchange (CIE), participate in APMs, and complete
required workflow training and onboarding - Communicate key directives, strategies, successes, and challenges from the RCN or
RHTO to Hub partners to support effective community-driven healthcare transformation.
