- Participate as a Hub partner or community referral partner, where applicable
- Help identify local health-related social needs and service gaps
- Support outreach, education, and connection to services for residents
- Coordinate with Hubs, providers, and other community partners to support local programs
- Participate in referral workflows, reporting, and training requirements, if receiving funding or formal Hub responsibilities
