What ToRCH Care Means for You
The Hub formally connects healthcare with community organizations that address food, housing, transportation, safety, and other non-clinical needs affecting people’s health. CBOs are not “add-ons”; they are full partners.
Community Based Organizations:
CBOs help shape priorities, accept referrals, and provide services that directly affect health outcomes. The Hub ensures your work is visible, coordinated, and supported. CBOs are full participants in the Leadership Board and program design process. Your expertise helps the community:
Understand root causes of poor health outcomes
Design programs that meet people where they are
Ensure services are accessible, culturally appropriate, and effective
Social Entities:
Schools, faith organizations, police and fire departments, and other trusted community entities often see needs first. The Hub gives these groups a clear, supported way to connect residents to needed help. Schools, faith organizations, police and fire departments, local court systems, and other community groups contribute by:
Sharing insight into emerging needs
Referring individuals to Hub-supported services
Participating in discussions about local priorities
How You Can Get Involved
- Participate as a Hub partner or community referral partner, where applicable
- Help identify local health-related social needs and service gaps
- Support outreach, education, and connection to services for residents
- Coordinate with Hubs, providers, and other community partners to support local programs
- Participate in referral workflows, reporting, and training requirements, if receiving funding or formal Hub responsibilities
