    access, sustainability, and outcomes
  • Use Hub-connected services as they become available, including care coordination,
    referrals, transportation supports, behavioral health access, community-based programs
  • Share feedback with local Hubs, providers, or community partners about barriers to care
  • Stay informed on new services, local programs, and community health resources
