The forms listed on this page are for exclusive use by Children’s Division employees that use DSS issued iPads in the field. Staff shall read and agree to the following stipulations prior to using these forms: 

  • Departmental iPads, internet and e-mail are to be used for work-related purposes only.
  • Personal identifications and passwords are not to be shared with anyone under any circumstances. You are responsible for all use associated with your assigned identification and password.
  • E-mail is not a secure method of transmitting information unless it is properly encrypted. Transmitting confidential or sensitive information via e-mail to entities that do not have a mo.gov e-mail address is only permitted if the e-mail is encrypted. To encrypt an e-mail, type [encrypt] somewhere in the subject line. REMEMBER: The e-mail subject line and the title of e-mail attachments are never encrypted; therefore, it is very important to never place client identifying information such as names, DCNs, SSNs, etc. in either the subject line or in titles of attachments being sent via e-mail.
  • iPads should never be left unattended. Log-off or use a lockup feature when you are not using your iPad.
  • Security incidents (e.g., lost or stolen iPad, unauthorized disclosures and/or breaches of security involving Protected Health Information (PHI), personal information, federal tax information, and social security information) must be reported to a DSS Security/Privacy Officer and to your supervisor as soon as possible. Staff should periodically review DSS Administrative Policy 6-100 for further details on all departmental information security policies. Compliance with DSS Administrative Policy 6-100 is mandatory for all personnel.
