Family Support Division Notification of HIPAA Disclosure

Jefferson City, MO—On October 24, 2017, the Missouri Department of Social Services, Family Support Division discovered a database mailing error that caused MO HealthNet Review Forms to be mailed to the wrong address. This incident affected 123 individuals and contained their full name, mailing address, and departmental client number (DCN). The Family Support Division has identified the cause for this error and corrected it. In compliance with federal and state privacy laws, notification letters were sent to the affected individuals on November 29, 2017, but a large percentage of those letters were returned to the Department as undeliverable.
