Money Follows the Person Semi-Annual Evaluation

The Missouri Money Follows the Person program collects data to report semi-annually on the effectiveness of the program, provide information for program improvement, and provide information about program operations and effectiveness. The following evaluation reports from the MO HealthNet Division include data related to the project’s implementation process and outcomes and examines points throughout the transition process from institutions to community settings.
